1. Deploy airway subpack 2. Unstow (equipment) 3. Contact surgeon 4. Restrain patient 5. Hyperventilate    5.1 Remove ET tube    5.2 Lubricate ET tube    5.3 Fill syringe with air    5.4 Check integrity of cuff    5.5 Deflate and leave connected 6. Extend and Check laryngoscope 7 - 10. Laryngoscopy 11. Hold tube in place, remove laryngoscope 12. Inflate cuff 13. Ventilate through ET tube 14. Verify placement 15. Reconfirm: use intubation bulb 16. Tape tube 17. Obtain vital signs 18. Monitor ECG 19. Follow respiratory support if available 20. Contact Surgeon ASAP
